This form is used when you sell your cottage or pass it on to the next generation. It should be delivered to the camp secretary as part of the ownership change process. Additional Documents When you put in your request for a stock or lease transfer to the Camp Secretary you also need to attach an Entity Certification form.  This form is for the proper identification of buyer(s) and seller(s)  The form is found here (PDF) and here (OpenOffice format.)

Both Cottages and Mobile homes are taxable and are accessed based on value by Waupaca County for personal property taxes.  Waupaca County requires the new owner to complete and return in a property tax form.   A copy of the property tax form is here (PDF) or here (MS Word format)
